Under the guidance of the Wellness Director (Project manager, Program champion, or Member Engagement Specialist), the Heart Healthy Ambassadors (HHA) will provide support to a group of participants in their journey toward improved health and blood pressure management.
Complete a series of online trainings through the Y’s Learning and Career Development Center.
Participant in meetings with the Project Manager or Program Champion on program goals.
Aid in implementation nutrition seminars once a month for participants.
Be able to assist with program promotions and participant recruitment activities.
Enroll new participants in the Blood Pressure Self-Monitoring Program.
Dedicate 3-8 office hours and provide 10-minute consultations and personalized weekly contact for the program by either txt, phone or email with encouraging support.
Willingness to travel to off-site locations in the Fredericksburg area.
Utilize REDCap web-based software to track participant information, Office Hours and Nutrition Education Seminar attendance, and blood pressure readings in an accurate and timely manner as assigned by the Program Champion.
Maintain participant confidentiality by managing program records and personal health information according to best practices.
Y-USA Healthy Heat Ambassador Certification
Interested in chronic disease prevention and able to provide on-going support to participants.
Ability to maintain a consistently positive, patient and non-judgmental attitude.
Effective at relationship-building, member engagement, motivating others, facilitating and encouraging participants to measure their blood pressure at home.
Understanding of/willingness to maintain fidelity to the evidence-based program model.
Comply with HEPA standards.
Apply strong Listen First skills.
Ability to communicate effectively with individuals in small groups and one-on-one settings.
Ability to guide behavior change without prescribing personal actions or solutions.
Ability to work with a diverse group of people of varying ages, ethnicity, life experiences, etc.
Knowledge of basic health, nutrition, and fitness principles.
Schedule that allows flexibility to facilitate office hours and education seminars.
Keep communication with the Project Manager and Program Champion.
CPR, AED and First Aid Certification (or will agree to become certified within three months of employment).
